    Use this form to register your company, club, or organization with WCRL for hosting WCRL-licensed events.  An Event License Application for the Group's first event must be submitted when filing to be listed as an Affiliated Group.

  • 2023 Rally Event Licensing Agreement - Application & Forms Packet (Fillable)
    This packet includes the Event Licensing Agreement & Application Form, Judges Panel & Class Schedule Form & Post-Trial Reporting Summary.  These forms are required to file application and host a licensed WCRL trial. Updated 01/03/2023.
  • 2023 Trial Score Reporting Sheet (Fillable)
    Complete a sheet for each class offered at a trial and submit to the WCRL office for input of results. The judge officiating the class must sign/initial each page to confirm correctness. Submit no later than 5 business days following the event. Updated 01/03/2023.

  • 2023 Post-Trial Reporting Summary

  • Request for Modification Form (one-page)
    At a trial, if handler or dog has a disability that prevents the team from performing a course or exercise as described, this form should be completed and submitted to the Desk Steward prior to the start of the class. Last updated 2/3/2018.
